Year 6 Permission Form

We are excited to inform you about the upcoming Big Bounce event on Thursday, July 11th. This event promises to be a fun-filled day for all participants.

We recommend that you accompany your child for the event, however, please be aware that children in Year 6 may attend this event without an accompanying adult, only if the below form is completed and returned by Tuesday 9th July. It is important to note that the school and the Parent and Friends Association (PFA) will not be responsible for the supervision of your child during this event.

I, the undersigned, permit my child (name stated above), to attend the Big Bounce event on Thursday, July 11th, without an accompanying adult. I understand and agree that the school and the PFA will not be held responsible for any incidents, injuries, or supervision during the event. I have made arrangements for my child’s transportation to and from the event.

I understand that without this competed form submission my child will not be granted access to the event without a supervising adult.
